STUDENT ACCOUNTS:
2008-2009 Policies and Procedures Manual - The Policies and Procedures Manual reviews Terms of Financial Aid, Student Account Policies, and Loan Information.
Deferred Payment Plan Form - The option for making monthly payments on a remaining balance due.
Financial Information Release - Financial information may be released to another individual, if the student consents by completing this form.
Health Insurance Waiver Form - Complete this form to provide Simpson University with your insurance policy information.
Health Insurance Request Form - Complete this form in order to request the Simpson University Accident and Sickness Insurance.
VERIFICATION
Dependent Verification Worksheet (2008-2009) - If you are classified by the federal government as a dependent student, print this PDF for income verification and submit to Simpson University with a copy of the federal tax return and W2's for both student and parent, additional tax information may be requested.
Independent Verification Worksheet (2008-2009) - If you are classified by the federal government as an independent student, print this PDF for income verification and submit to Simpson University with a copy of the federal tax return and W2's, additional tax information may be requested.
Special Conditions Worksheet (2008-2009) - In accordance with the guidelines established by the federal needs analysis methodology, a student may provide information to assist Simpson University to assess a student's ability to pay for their education. There are four main circumstances that may be considered (Income Reduction, Nonrecurring Income, Unusual Medical and Dental Expenses, or Elementary and Secondary Tuition costs; Child Care costs and Dependent Care costs). Be sure to effectively explain the circumstances you are facing with appropriate supporting documentation.
